In consideration for the right to participate, by execution of this form you release and discharge the Silver Reef Hotel Casino Spa and the Lummi Indian Nation and Tribe, their officers, directors, agents, employees, representatives and anyone else connected with the above of and from any and all known or unknown damages, injuries, losses, judgements and/or claims from any cause whatsoever that may be suffered by any entrant, spectator or their passenger(s) and/or to his or her property. Further, each entrant expressly agrees to indemnify all of the foregoing entities, persons or bodies of and from all liability occasioned or resulting from conduct of entries, spectators, workers or any participants. I hereby authorize Silver Reef Hotel Casino Spa, it’s advertising agency, agents, nominees, licensees, and their successors and assigns, those acting with its authority and those for whom it is acting to use any still, video, or motion picture of me, with or without my name and information about me, in any way Silver Reef Hotel Casino Spa may desire for purposes of trade, advertising, publicity or promotion of any kind with or without compensation.

Decision of executive management will be final for stock, custom or other classifications and/or the combining, adding or eliminating of such classifications. This will be based largely on pre-registration. Any appeals of assigned classes must be brought to the attention of the Silver Reef Hotel Casino Spa executive management before 10am day of show.
